Brief Description of the Service(s) You Offer: 
With a commitment to a strength based, family centered model, we are dedicated to supporting families who have children that are diagnosed with Severe Emotional Disability (SED).  By providing an evidenced-based goal oriented program, we will help develop and build upon the family’s existing skills. 
IN HOME FAMILY PROGRAMING: 
In valuing your decision making abilities we will establish an environment of open communication. 
After discussing your family’s needs and concerns, we will develop a plan of service for your child. 
The plan developed by the family and the support team will be implemented by modeling effective research-based tools and programs.  
TANDEM GROUP CLASSES: 
Classes will be ran simultaneously for you and your child to accommodate your time. 
Family recreational events 
Family fun events are planned periodically for all to enjoy. 
OVER THE PHONE SUPPORT: 
Phone support will be provided between the hours of 9am and 5pm to families receiving in home family programming . 
FAMILY ADVOCACY/SUPPORT: 
With the use of collaborating agencies and community resources, we will support you in advocating for your family to insure that you are receiving the programming and aid you deserve. 
SOCIAL SKILLS: 
After completing a behavior evaluation, the team will choose specific skills to improve. Some of these skills may include problem solving, improving peer and family relationships, anger management, raising self-esteem, health and hygiene.
The program coordinator goes into the home and does behavior modification with the family; and the behavior specialist takes the child out into the community to work on social skills and to give the parents a break for a few hours a week.
Parent Roundtables
Parent Roundtables offer parent education and is a parent support group that is done one time a month

Currently all referrals for the Supporting Families Program are through the Fond du Lac Department of Social Services.  If there are any questions on how to be referred to the Supporting Families Program please feel free to contact us.

9:00 am – 6:00 pm   Monday - Friday 

Other Information That Should be Included to Describe Your Organization: How our services work: 
Each family will work with a family specialist. The Family specialist has a small case load which allows them to work intensively with each family. The bulk of the work however is done by the family. The intensive portion of the program usually lasts 8 weeks followed 6 to 8 months of less intensive support.
